All ex-servicemen and their widows, who are residents of Chandigarh (UT) should register themselves with Zila Sainik Welfare Office. After the registration they will be issued an identity card.
Registration Procedure. The procedure for registration is as under:-
(a) The ex-serviceman or widow will have to fill the registration form. The format of the form is available at Download section.
(b) Along with the above form the following documents will be required for the registration and issue of identity card:-
- Two photographs (passport size)
- Copy of Pension Pay Order and Dissability PPO (if dissabled)
- Copy of Service Particulars Book for officers/ Copy of Retirement Order and Discharge Book for the PBOR.
- Copy of residence proof. Any of the following documents may be attached as proof of residence:-
- Domicile Certificate
- Certificate issued by SDM
- Voter Identity Card
- Passport
- Allotment Letter with Possession Certificate for Govt. Accommodation in Chandigarh
- Driving Licence
- Telephone Bill of BSNL
- Electricity/ Water Bill of Chandigarh Administration
(c) Follwoing documents are also required .
- Aadhaar Card (self and dependents)
- PAN card
- Contact number and email ID
In addition to the documents mentioned above the widows need to submit the following:-
(a) Copy of death certificate of the ex-serviceman
(b) Identity card issued to the deceased ex-serviceman
