All Ex-Servicemen or their Widow and their eligible Dependents, who are residents of Chandigarh (UT) are suppose to register themselves with the Zila Sainik Welfare Office. After registration, they will be issued with an Ex Serviceman Identity Card.
The procedure for registration is as under:
The Ex-Servicemen or their Widow or their eligible Dependents must fill out the Registration Form held with Zila Sainik Welfare Office the same is also available in the Download Section of this site.
Along with the above form the following documents are required for the registration and issue of Identity Card:

Ration Card / Voting Card / Residence Certificate / Passport / Latest Electricity Bill (Chandigarh Administration) / Power of Attorney / Allotment Letter with Possession Certificate for Govt. Accommodation / Flat / Govt. House in Chandigarh. |
| 5 | NOC from the parent ZSWO in case, Ex-serviceman/Widow belongs to other state or Permanent Address in Discharge book /Service book is other state other than Chandigarh (U.T.).. |
| 6 | Photocopy of Aadhaar & PAN Cards for establishing identity of individual. |
| 7 | Copy of DDR/FIR in case of loss of Identity Card with endorsement from police station after one month of DDR/FIR. |
| 8 | Original Old Ex-servicemen/Widow Identity Card (in case of duplicate or widow identity card issuance). |

Latest two stamp-size photographs after retirement in civil dress (H 2.5cm × W 2.2cm),
without spectacles, headgear, cap, scarf, etc.
For Dependent Identity Card – photographs with Blue Background only. |
